You can upload the signed authorization form when placing your order or after the order has been placed.
During Order Instructions:
(1) Login and select the ORDER REPORTS tab to place an order.
(2) Under Applicant: Create Profile you will see an option to Upload Documents.
After Order Instructions:
(1) Login to Account and click on the "IN PROGRESS" icon.
(2) Select your Applicant.
(3) Under the Applicants DOB (Date of Birth) there is a link called VIEW REPORT OPTIONS - select it.
(4) You will now see a link called "UPLOAD DOCUMENTS FOR THIS REPORT".
(5) You can now upload the authorization form from your computer.
